Service Fees

Mountain Hope Good Shepherd Clinic is a faith based charitable clinic which means we strive to keep fees for services at an affordable cost however, we are not a free clinic. Patients will pay for services at the time of visit based on our sliding-fee scale.

Our Clinic participates in several state funded programs that require financial eligibility screening to receive assistance and services through these programs. Fees for Clinic services are based on a sliding fee scale determined by household size and income. We follow the Federal Poverty Level Guidelines and proof of income is required to determine eligibility. 

What is Needed for Proof of Income

  • Pay stubs from an employer. Please include all pay stubs for the most recent 4 full weeks in which pay was received.
  • IRS tax filings (IRS Form 1040s) for the most recently filed tax year. Other official IRS forms which show income, such as W-2s and 1099s, are also acceptable.
  • Unemployment Insurance/Compensation and /or Social Security award letter(s)

Our Clinic engages other community healthcare entities that may offer discounts for services. Our providers may refer you to other specialists or for testing at another healthcare location. Please note our Clinic sliding scale does not apply to these services.
