Crossroads to Hope Through Recovery Support

Program Includes:

• Screening for Mental Health and Substance Use Disorders
• Support from a Community Health Worker
• Individual Therapy
• Mental Health Medication Management
• Family Education and Support
• Connection to Primary Care and Dental Care
• Culturally and Linguistically Appropriate Services

Those eligible include individuals and/or families/support systems who have been impacted by the Opioid Crisis. Individuals must be uninsured and live or work in Sevier County, TN.
