Our Clinic participates in several state funded programs that require financial eligibility screening to receive assistance and services through these programs. Fees for Clinic services are based on a sliding fee scale determined by household size and income. We follow the Federal Poverty Level Guidelines and proof of income is required to determine eligibility.\u00a0<\/p>

What is Needed for Proof of Income:<\/strong><\/p>

    Our Clinic engages other community healthcare entities that may offer discounts for services. Our providers may refer you to other specialists or for testing at another healthcare location. Please note our Clinic sliding scale does not apply to these services.<\/p>
